In this role you will be responsible for leading activities of the project team independently. You will assist with identifying appropriate resources needed, and developing schedules to ensure time completion of the project. You will coordinate the activities of the team by becoming familiar with system scope and project objectives, as well as the role and function of each team member. You will take leadership of project teams, as well as coach the development of other staff and implement multiple complex projects.
Develops project plans, goals and objectives in collaboration with project and department leadership.
Provides professional and/or technical leadership in the execution of day-to-day project activities.
Communicates within and across department to maximize information sharing around progress, needs, inter-dependencies and accomplishments.
Assures successful implementation of people, process and systems changes to support redefined service delivery model.
Participates in institutional task forces and other special committees related to the achievement of goals and objective.

Vanderbilt Discharge and Transitions is an innovative and cutting-edge program that supports Vanderbilt University Hospital's mission. The program focuses on development, implementation and oversight of standard guidelines and innovative alternative care delivery models to ensure excellent, patient centered, and reliable care delivery for patients during time of discharge and transitions out of the hospital. The program oversees all efforts related to hospital discharge, transition of care and readmission reduction for adult patients. VDAT is a start-up, allowing the staff to set the course and culture as well as providing the opportunity to play a key role in this venture as part of Vanderbilt University Medical Center.
